Three-Step Integrative Treatment of a Post-Surgical Hypertrophic Scar Using Subcision, Q-switched Nd:YAG Laser, and PDRN Pharmacopuncture: A Case Report

Objectives : This case report presents a three-step integrative treatment protocol for hypertrophic scarring involving subcision (dochim), Q-switched Nd:YAG laser therapy, and polydeoxyribonucleotide (PDRN) pharmacopuncture. Methods : A 26-year-old male with a post-surgical facial hypertrophic scar received sequential treatment: subcision to release fibrotic adhesions, 1064-㎚ Nd:YAG laser therapy (spot size 7㎜; fluence 2.07–2.59J/㎠; frequency 5–7㎐) until delayed petechiae appeared, and 2 mL of PDRN pharmacopuncture to promote VEGF expression and fibroblast activation. Outcomes were assessed using SBSES, QIS, and a Likert scale for patient satisfaction. Results : After two months, SBSES improved from 3 to 1, QIS from 3 to 4, and patient satisfaction from 4 to 5. No adverse events were observed. Conclusions : This case suggests that combining subcision, Nd:YAG laser, and PDRN pharmacopuncture may offer an effective integrative strategy for hypertrophic scar management. Further studies are needed to validate these findings

Siyeon Kim is currently a senior (4th year) student at the College of Korean Medicine, Sangji University, Republic of Korea. She has cultivated a strong academic interest in dermatological and aesthetic applications of traditional Korean medicine, particularly through integrative protocols that combine acupuncture, herbal pharmacology, and laser-based therapies. Kim has actively participated in clinical volunteering, academic clubs, and campus-led research initiatives. She also curates educational content for the general public via her Instagram platform @yeoni_hani, with a mission to enhance the accessibility and scientific perception of Korean medicine among younger generations. Her most recent academic work, a case report titled “A Case of Hypertrophic Scar Treated by a Three-Step Integrative Protocol Using Dochim Subcision, Q-switched Nd:YAG Laser, and PDRN Pharmacopuncture,” was published in the Journal of Korean Medicine Ophthalmology, Otolaryngology & Dermatology, where she was the first author. Through this work, she aimed to contribute clinical evidence that supports integrative scar treatment approaches within Korean medicine. Kim plans to further explore the intersection of evidence-based traditional therapies and modern technology, particularly in the fields of scar care, cosmetic dermatology, and regenerative skin treatments.
